Helltide
A Helltide is Diablo 4's region-wide event in which Hell bleeds into Sanctuary: monsters drop Cinders that open Helltide chests, killing raises a Threat meter that brings a Hellborne, and three Baneful Hearts at the Accursed Ritual summon the Blood Maiden. Since Season 3 (Jan 2024) Helltides are active all the time except a 5-minute rest each hour.
A Helltide is Diablo 4's region-wide event in which Hell spills into Sanctuary: the monsters in that region grow stronger and drop Cinders, a currency that opens Helltide chests, and since Season 4's "Helltide Reborn" a Threat meter brings a Hellborne ambush and three Baneful Hearts at the Accursed Ritual summon the Blood Maiden. Helltides have been part of the Endgame since launch, and since Season of the Construct (Jan 2024) they are always active, save for a 5-minute rest once per hour. Since Lord of Hatred you can also run one as part of War Plans, which teleport you there: IGN's reviewer notes in this clip that you no longer need to "manually walk over to helltide events".

When and where Helltides happen
A Helltide covers one region of Sanctuary at a time and changes its ground and its monsters: Blizzard's first description says Lilith's servants are "empowered, having their difficulty increased, but dropping loot worthy of the danger" (Blizzard, Sep 19, 2022). At launch they opened only in World Tier III, "Nightmare"; Season 4 opened them to World Tiers I and II with lighter monster density and no roaming bosses, and patch 2.0 replaced World Tiers with the difficulties described in Difficulty and Torment. Season of the Construct (Jan 23, 2024) made them nearly constant: since then Helltides are "always active, save for a 5-minute rest once per hour", so the answer to "when is the next Helltide" is almost always now. The Regions page describes the regions a Helltide can cover.
Cinders and Helltide chests
Monsters in a Helltide drop Cinders, which you spend to open the Helltide chests scattered across the region. At launch each chest held rewards for a single item slot, such as Torso, Legs or a two-handed weapon (Blizzard, Sep 19, 2022). Hoarding Cinders is a risk: "should you fall in battle, they'll drop and must be reclaimed". Helltides also hold Tortured Gifts, Chests of Mystery and Doomsayers; since Season 4 Doomsayers drop their rewards on death instead of a Cinder-cost chest, and Doomsayers in Helltides are a guaranteed source of Infernal Compasses (Blizzard, Jul 30, 2024). Helltide chests were one of the first sources of Runeshards for The Pit, and patch 3.1.0 (Jun 30, 2026) improved their loot again (Blizzard).
Threat and the Hellborne
Season 4: Loot Reborn (May 14, 2024) rebuilt Helltides around Threat, and Blizzard made the change permanent on both realms (Blizzard, Apr 30, 2024). Killing monsters and opening Tortured Gifts raises your Threat through three levels, each bringing more frequent and dangerous ambushes. At level 3 you are Hell-Marked: a frenzy of packed monsters attacks, and if you survive, a Hellborne comes for you, "one of Sanctuary's most formidable fallen warriors", which can take the form of any class. Your Threat resets after the Hellborne ambush or when you die.
Baneful Hearts and the Blood Maiden
Baneful Hearts come from Tortured Gifts and from slain Hellborne. Bring three to the Accursed Ritual, a scene of "shocking gore" in the Helltide region, to start the ritual: a swarm of demons attacks first, then the Blood Maiden arrives, "an immensely powerful demon who offers great reward upon her death" (Blizzard, Apr 30, 2024). Any player who adds a Heart gets increased rewards as a contribution bonus. For the other bosses of the game see Bosses.
Seasonal twists
Seasons often change Helltides. In Season of Divine Intervention (Dec 11, 2025) the Lesser Evils invaded the Seasonal Realm's game modes, and Duriel, King of Maggots, took Helltide: he could be summoned instead of the Blood Maiden, Pangs of Duriel replaced the Hellborne at maximum Threat, and meteors fell as packs of maggots (Blizzard, Dec 3, 2025). The season's trailer shows the words "Rage against the Burning Hells as the Lesser Evils invade" here, then a giant red demon in the snow. Earlier, Season 4 sold the Profane Mindcage, an elixir that raised Helltide monsters by 10 levels for more Cinders, but only until that season ended; its Iron Wolves events stayed.
In Season of Hell's Legacy (since Sep 15, 2026), the smallest Waking Nightmares appear in the open world: Blizzard's designers explain in this clip that while "you're playing through hell tide" a portal can open into a one-room fight, a wave event or a boss from an earlier Diablo game. Diablo's Splinter of Terror, one of the season's three Splinters of Evil, modifies Helltide and Infernal Hordes through War Plans.
Helltides in War Plans
Inside War Plans a Helltide is one node of your chain, and its activity tree changes how it plays; as Blizzard's developers put it in this clip, the trees can "change the rewards, tweak the monster spawns, even bring features from one activity into another". Blizzard's own example is Duriel's Invasion, which replaces the Blood Maiden with Duriel (Blizzard, Feb 11, 2026); patch notes also name a Hellmouth node that spawns Bloodwyrms and maggots (Blizzard). Since patch 3.1.0 the Helltide objective is a set number of Cinders rather than a number of chests, and War Plans offer options for longer Helltides (Blizzard, Jun 23, 2026).
Frequently asked questions
When is the next Helltide in Diablo 4? Almost always now: since Season 3 (Jan 2024), Diablo 4's Helltides are always active except for a 5-minute rest once per hour, in one region at a time shown on the map.
How do you summon the Blood Maiden in Diablo 4? Collect three Baneful Hearts from Tortured Gifts and Hellborne during a Diablo 4 Helltide, then bring them to the Accursed Ritual; after a wave of demons the Blood Maiden appears.
What happens to Cinders when you die in Diablo 4? When you die in a Diablo 4 Helltide your Cinders drop where you fell and must be picked up again, so spend them on Helltide chests before a risky fight.
What is Threat in a Diablo 4 Helltide? Threat is a meter in Diablo 4's Helltides that rises as you kill and open Tortured Gifts; at level 3 you become Hell-Marked, survive an ambush and then face a Hellborne.
Do you need Lord of Hatred for Helltides in Diablo 4? No. Helltides are part of the Diablo 4 base game on both realms; only running them inside War Plans needs Lord of Hatred.
What are Helltide chests in Diablo 4? Helltide chests are chests scattered through a Diablo 4 Helltide region that you open with Cinders dropped by the Helltide's monsters.

EndgameDiablo 4's endgame in 2026 is built around War Plans in Temis, which chain up to five runs of the Pit, Infernal Hordes, Helltides, Nightmare Dungeons, Lair Bosses and the Kurast Undercity, with Echoing Hatred, Torment I to XII and each season's own activities on top. War Plans and Echoing Hatred need Lord of Hatred; the activities themselves and the Torment levels are free for every owner of the base game. -
Lord of HatredLord of Hatred is Diablo 4's second expansion, released Apr 28, 2026 (Apr 27 at 4 p.m. PT in the Americas): the Paladin and Warlock classes, the region of Skovos and the final campaign against Mephisto, with War Plans, Echoing Hatred, the Horadric Cube and Talismans. Every edition includes Vessel of Hatred; Standard costs $39.99. -
War PlansWar Plans are Lord of Hatred's endgame system: at the Command Table in Temis you chain up to five activities from six modes (the Pit, Infernal Hordes, Helltides, Nightmare Dungeons, Lair Bosses and the Kurast Undercity), claim rewards from the War Chest and level a modifier tree for each activity. Since Season of Hell's Legacy (Sep 15, 2026), War Plans progress is shared by your characters in the same partition. -
Difficulty and TormentDiablo 4 has four standard difficulties (Normal, Hard, Expert and Penitent) and twelve Torment levels, Torment I to XII, since Lord of Hatred (Apr 27, 2026), up from Torment I to IV in Vessel of Hatred's patch 2.0; the original World Tiers were retired in Oct 2024. Torment levels unlock by clearing Pit tiers and give more gold, experience and better drops. -
RegionsDiablo 4's Sanctuary has seven regions in one open world: Fractured Peaks, Scosglen, Dry Steppes, Kehjistan and Hawezar in the base game, Nahantu from Vessel of Hatred and Skovos from Lord of Hatred. Skovos's capital, Temis, is the endgame hub. -
Infernal HordesInfernal Hordes is Diablo 4's wave mode, added in Season 5 (Aug 6, 2024) and permanent on both realms: open it with an Infernal Compass, survive 60-second waves, pick Infernal Offers (a boon with a bane), collect Burning Aether, then beat the Fell Council or Bartuc, Lord of Chaos, and spend the Aether on the Spoils of Hell.
