Lilith
Lilith, the Daughter of Hatred and daughter of Mephisto, co-created Sanctuary with the angel Inarius and is the villain of Diablo 4's base campaign, defeated in the Cathedral of Hatred; she returns as a reluctant ally in Lord of Hatred. Caroline Faber voices her, and Echo of Lilith is the base game's pinnacle boss.
Lilith is the Daughter of Hatred, the demon daughter of Mephisto who made the world of Sanctuary with the angel Inarius and became the mother of humanity, and she is the villain of Diablo 4's base campaign: summoned back from the Void, she tries to forge Sanctuary into a weapon against the Burning Hells until the Wanderer defeats her in the Cathedral of Hatred. She is not gone for good. In Lord of Hatred (2026) the Wanderer needs her help against her father, and Blizzard's site says that "Lilith herself is the only way to reach" the last hope of stopping him. At the Diablo 30th Anniversary Spotlight of Feb 11, 2026, Blizzard introduced the expansion's island of Skovos as "the birthplace of humanity, where Lilith and Inarius began it all", with the Blessed Mother and the Heavenly Father carved into the marble of its capital, Temis. Caroline Faber voices her. Spoilers for the base game and Lord of Hatred follow.

Who Lilith is
"Skovos is where Lilith and Inarius created mankind," Lorath tells the Wanderer in Lord of Hatred's launch trailer. Blizzard's own account, published by Windows Central as "Those We've Lost", gives her titles as "Daughter of Hatred, Creator of Sanctuary, and Mother of All Humankind": she and Inarius made Sanctuary as a hidden escape from the Eternal Conflict between the High Heavens and the Burning Hells, and on the Skovos Isles they gave birth to humanity, starting with their son Rathma. Inarius came to fear their children's power, and the two fought; in the end he banished her to the Void. Since then, Blizzard writes, she "has remained absolutely devoted to her children, willing to commit terrible atrocities in the name of their survival".
Her return in the base game
Lilith is the villain of the 2023 campaign, told in a prologue, six acts and an epilogue (Story). Elias, Lorath Nahr's former apprentice, brought her back from the Void with a ritual and stayed at her side. Blizzard's recap says she returned with a promise "to protect humanity from the Eternal Conflict", but only by claiming the world "as her own, to forge it into a weapon for her to conquer the Burning Hells". She unleashed suffering on Sanctuary "not to torture or kill, but to make her children strong enough to face the Burning Hells", and she sought to consume her father, Mephisto, and take his power. The Wanderer, marked by her blood, followed her with Lorath and Neyrelle across Sanctuary and into Hell. There she defeated Inarius, "running him through with his own spear before tearing his wings from his back". The heroes trapped Mephisto in a soulstone before she could consume him, then defeated her in the Cathedral of Hatred. As she turned to dust she warned: "Without me, you would have no victory." Blizzard's own aside, years later: "And spoiler, she was right."
Lord of Hatred
Lilith returns in Lord of Hatred, this time against her father. Near the end of the expansion's launch trailer a lone figure stands in a ring of red on dark ground, a horned and crowned figure faces an armored knight, and a voice answers a hero's "I cannot stop his schemes alone" with "No. You need me." Blizzard's description of the expansion says Mephisto must be stopped "before he reaches the Pools of Creation and bathes the world in hatred", and that "Lilith herself is the only way to reach" the beacon of hope. Grimdark Magazine's review says she is awakened through the Wanderer's blood link and "begrudgingly agrees to help". Blizzard's account of the ending says Mephisto fell "with the help of his daughter Lilith and the weapon she had made from her own body": the Wanderer stabbed him through the heart with it and banished him into the Void. What became of Lilith herself is on Lord of Hatred Ending, which keeps to Blizzard's published text.
The developers count that bond as the heart of the expansion. At BlizzCon 2026, a member of the Diablo 5 team said what made Lord of Hatred "so amazing to experience was the depth of that relationship that you get to build with Lilith and with Mephisto", because by then the Wanderer is "central to the story of these eternal characters". Diablo 5 builds the same kind of bond with Diablo from its start (see Diablo 5 on its own viki).
Echo of Lilith
Echo of Lilith is the base game's pinnacle boss, an endgame return of the Daughter of Hatred. Blizzard's patch notes for 2.0 (Nov 15, 2024) say defeating her rewards a Resplendent Spark, the material used to craft Mythic Uniques, and mention her power to destroy the floor of the arena. Lord of Hatred's own pinnacle is Echo of Mephisto. The other bosses are on Bosses.
In marketing and music
Lilith was the face of Diablo 4's launch. Activision Blizzard said on Jun 12, 2023 that she "showed up on the streets of Sydney and Taipei, appeared at a baseball game in Anaheim" and appeared on buildings in Las Vegas and New York, and that "Lilith (Diablo IV Anthem)" by Halsey and Suga of BTS, released on Jun 5, 2023, "topped the charts in more than 70 countries on iTunes". Blizzard's developers still hold her up as a model villain: on the Diablo 5 panel at BlizzCon 2026, a developer said the team loves "a good morally gray character" that makes players ask "are they right?", naming "Sylvanas" and "Lilith" as Blizzard's best, before deciding that Diablo himself is not that kind of villain.
