Story

Diablo 4's story is the "Age of Hatred": Lilith returns to Sanctuary fifty years after Diablo 3, the Wanderer and Lorath defeat her and seal Mephisto in a soulstone, Neyrelle carries it to Nahantu where Mephisto takes the prophet Akarat's body, and Lord of Hatred ends with Mephisto banished to the Void. The seasons since then follow the dead returning to life, and Diablo 5 is set 100 years later.

Diablo 4's story is the "Age of Hatred", told across the base game (2023) and two expansions: Lilith, the mother of humanity, returns to Sanctuary fifty years after Diablo 3, the Wanderer and the Horadrim Lorath Nahr defeat her and trap her father Mephisto in a soulstone, Neyrelle carries the stone to Nahantu where Mephisto takes the prophet Akarat's body (Vessel of Hatred), and Lord of Hatred ends with Mephisto stabbed through the heart and banished into the Void. Blizzard's developers describe the last chapter at the Diablo 30th Anniversary Spotlight as "the eternal conflict between angels and demons, with humanity stuck in the center", a tale where "Mephisto's shadow looms even larger": "an unfolding saga of hatred, treachery, and the search for salvation". This page follows Blizzard's own recaps; every section after the setting holds spoilers, and the last chapter's ending has its own page, Lord of Hatred Ending.

Setting and timeline

Diablo 4 is set in Sanctuary, the world the angel Inarius and the demon Lilith made as a refuge from the Eternal Conflict between the High Heavens and the Burning Hells; on the Skovos Isles they gave birth to humanity, starting with their son Rathma (Windows Central, reproducing Blizzard's "Those We've Lost", Jun 25, 2026). Blizzard returns there at the end: Skovos, its developers say at the Spotlight, is "the birthplace of humanity, where Lilith and Inarius began it all", with the Blessed Mother and the Heavenly Father carved into the marble of its capital, Temis. Lilith is Mephisto's daughter, the "Daughter of Hatred"; Inarius later banished her to the Void.

When the game opens, "fifty years have passed since the Reaping and the defeat of Malthael," the events of Diablo 3: Reaper of Souls, "Fifty years of fragile peace, of rebuilding, of forgetting," in the words of Blizzard's recap "The Wanderer's Burden: A Journey Through Hatred" (Windows Central, Feb 4, 2026). You need no earlier game to follow it. The places are on Regions, the people on Characters.

The base campaign: Lilith returns (2023)

Spoilers from here on. A ritual tears Lilith out of the Void, and she comes back with a promise: to protect humanity, but "only by claiming the world as her own, to forge it into a weapon for her to conquer the Burning Hells" (Blizzard's recap). The ritual was the work of Elias, once Lorath Nahr's apprentice in the Horadrim, who had promised his own head to the Tree of Whispers. The player is the Wanderer, "a hero marked by the blood of Lilith", helped by Lorath and the young scholar Neyrelle. At the same time Inarius, back from millennia of torment in the Hells, hunts Lilith to win his way back into Heaven, and kills his son Rathma, the first Necromancer, for refusing to give up the key that seals the Gates of Hell.

The chase goes through every region and ends in the Burning Hells. There Lilith kills Inarius, "running him through with his own spear before tearing his wings from his back" (Windows Central, Blizzard text). The heroes reach Mephisto first and imprison him in a soulstone before Lilith can consume him, then defeat her in the Cathedral of Hatred. She turns to dust with a warning: "Without me, you would have no victory." Neyrelle walks away with the soulstone. Siliconera lists the campaign as a prologue, six acts and an epilogue, from "Wandering" to "From the Wound Spilled" (Siliconera); Blizzard's own pages read here do not list them. Lilith has her own page, and she returns in the endgame as the boss Echo of Lilith (Bosses).

Vessel of Hatred: Mephisto takes Akarat (2024)

The first expansion follows the soulstone. Neyrelle carries it into the jungles of Nahantu, looking for the lost tomb of the prophet Akarat, founder of the Zakarum faith, while the stone torments her with visions; the Wanderer follows with Eru, a Spiritborn of Nahantu. In the tomb they lay the soulstone on Akarat's body, which cuts Neyrelle's link to Mephisto. Then Eru betrays them, takes the body and the stone ("I've done my part... See that you do yours."), and at the temple city of Travincal, Mephisto's corruption wraps Akarat's body into a new vessel (Windows Central, Blizzard text). Mephisto takes Akarat's body, not Eru's. IGN's review of the next expansion begins on that ending, which left its reviewer "dangling off a narrative cliff". Neyrelle and Mephisto have their own pages.

Lord of Hatred: the last stand (2026)

Lord of Hatred is the "grand face-off with Mephisto", Blizzard said when it opened the 30th anniversary year. Its setup in Blizzard's words: Mephisto's power "stirs once more, spreading corruption toward the sacred isles of Skovos"; the Wanderer must stop him "before he reaches the Pools of Creation and bathes the world in hatred", and "Lilith herself is the only way to reach" the one beacon of hope (Blizzard).

Mephisto arrives on Skovos wearing Akarat's body, as a holy prophet leading pilgrims and promising to save its people from the Age of Hatred. The launch trailer opens on it: a woman the captions call the Amazon Queen announces that "a pilgrim comes to our shores", the bearded figure the captions name Akarat says, "It is in our nature to hate, and that is why I've brought you here," and a voice warns, "He's a demon." A timer runs through the campaign, an eclipse Mephisto brings on (PC Gamer, Apr 21, 2026). Tyrael, "ex-angel, founder of the Horadrim", is back at the Wanderer's side (Grimdark Magazine, Jun 29, 2026).

The ending, in brief: Neyrelle, Lorath and the Amazon High Queen Adreona die along the way, and with Lilith's help and "the weapon she had made from her own body", Mephisto is "stabbed through the heart" and "banished screaming into the Void between realms", unable to return to Hell (Windows Central, Blizzard text). The full account is on Lord of Hatred Ending.

What made it work, a Diablo 5 developer said at BlizzCon 2026, was "the depth of that relationship that you get to build with Lilith" and with Mephisto: "By that point in the story, you're not just a wanderer anymore. You are central to the story of these eternal characters." Brent Gibson put it in one line to PC Gamer: "We feel like we told the Story of Hatred. We had a great conclusion with Lord of Hatred with what we have with Mephisto, and we are moving towards telling the Story of Terror" (PC Gamer, Sep 12, 2026).

After Lord of Hatred: the dead return

The story goes on in the seasons. Season of Death Awakening (Jun 30, 2026) "descends in the wake of Mephisto's defeat", Blizzard wrote, adding: "The Age of Hatred has ended" (Blizzard, Jun 23, 2026). Its gameplay trailer opens on a sermon of the new enemy, a death cult: "Brothers and sisters, to live is to suffer," and "Only in death will we find salvation."

Blizzard now tells one story across the seasons. At BlizzCon 2026's What's Next panel, the live-service team explains that after Lord of Hatred the team can "chart a little bit of a story that's based on Lord of Hatred": a death cult opening ruptures to Pandemonium, then the dead coming back to life in Season of Hell's Legacy, and in the seasons leading up to the Amazon's release, a chance to "confront this death cult before they consume sanctuary and ultimately confront the aspect of death" (Blizzard, Sep 13, 2026).

Season of Hell's Legacy, the 15th season, brings back Deckard Cain. On the What's Next panel, Live Service Design Director Dan Tanguay says the dead are "starting to come back to life", and the panel explains that Cain returns "as a return soul", "really on borrowed time", thinking it is still the year he died, and sends you to redeem three lost souls through the nightmares of Diablo, Baal and Mephisto (Season of Hell's Legacy). Blizzard says no more Diablo 4 expansions are currently planned (Brent Gibson, Sep 2026); the next new class, the Amazon, comes from Skovos in the first half of 2027.

What comes next: Diablo 5

Diablo 5, announced at BlizzCon on Sep 12, 2026 for Spring 2029, is set about 100 years after Diablo 4. At its panel the Diablo 5 team says the victory over Mephisto mattered: "what you did in defeating Mephisto, it worked. There was a golden age of sanctuary." Then Diablo conquered it: the new game opens seven years after he took Westmarch, in a Sanctuary where "Diablo has won" (Windows Central, Sep 13, 2026). It is not a direct sequel to Lord of Hatred's ending; the bridge between the two games is on Diablo 5.
