Lord of Hatred Ending
Lord of Hatred ends with Mephisto, disguised on Skovos as the prophet Akarat, stabbed through the heart with a weapon Lilith made from her own body and banished into the Void between realms, unable to return to Hell; Neyrelle, Lorath and Queen Adreona die along the way. This page is all spoilers.
Lord of Hatred ends with Mephisto, who came to Skovos disguised as the prophet Akarat, stabbed through the heart with a weapon Lilith made from her own body and "banished screaming into the Void between realms", "Not dead, not alive, and unable to return to Hell"; Neyrelle, Lorath Nahr and the Amazon High Queen Adreona die along the way. That is Blizzard's own account, from its feature "Those We've Lost", which Windows Central published on Jun 25, 2026 (Windows Central). This whole page is spoilers for Diablo 4's Lord of Hatred campaign. IGN's review calls it "a wonderfully satisfying conclusion to the Mephisto saga". It closes the Age of Hatred that began with Lilith in 2023; the whole saga is on Story.

The setup
At the end of Vessel of Hatred, Mephisto's corruption took the body of the prophet Akarat, founder of the Zakarum faith, as its new vessel. In Lord of Hatred he uses it as a disguise: "the holy prophet Akarat" arrives on Skovos with scores of pilgrims and promises Queen Adreona that he will save her people from the Age of Hatred, "But in reality, he was Hatred Incarnate" (Blizzard's text). The launch trailer opens on that arrival: the woman the captions call the Amazon Queen says "a pilgrim comes to our shores", and the bearded Akarat says, "It is in our nature to hate, and that is why I've brought you here." Blizzard's summary of the campaign: stop Mephisto "before he reaches the Pools of Creation and bathes the world in hatred", with "Lilith herself" the only way to reach the beacon of hope (Blizzard). An eclipse brought on by Mephisto is the campaign's clock (PC Gamer, Apr 21, 2026).
Does Neyrelle die?
Yes. Neyrelle, who carried Mephisto's soulstone after the base game, never stopped looking for a way to end him for good. Blizzard's text: "just as she found it and sent word to her friends, Mephisto found her. And at last, took her life. She died as a true Horadrim, wearing the talisman given to her by Lorath." Blizzard posted the expansion's opening cinematic with a spoiler warning; in this stretch its frames show a young dark-haired woman by candlelight among old drawings and a sun-shaped symbol, a pigeon at a ledge, and a long-haired, gray-bearded old man in pale green light. The cinematic itself names no one.
Does Lorath die?
Yes. Lorath Nahr, "the last Horadrim of the modern age", was the only one to speak the truth to the people of Skovos when Mephisto came disguised as Akarat. He helped win a victory in Athulua and found "the weapon that would stop Mephisto for good", but did not live to see it used: Queen Adreona, "a woman he had loved", cut him down, and his head was taken to the Tree of Whispers, to which he had once bargained it. "His soul would have languished there forever, but his allies would not permit it": they set fire to the Tree and laid him to rest (Blizzard's text, via Windows Central).
What happens to Queen Adreona?
Adreona, High Queen of the Amazons and protector of the Skovos Isles, had once worked beside Lorath when he came to Skovos looking for lost Horadrim. Mephisto "tricked and coerced Adreona into believing his lies, slowly turning her against her own people" and against Lorath. Convinced he had become her enemy, she killed him "in cold blood", which left his allies "no choice but to strike her down on the cliffs of Mount Hefaetrus" (Blizzard's text). Her Amazons return to Diablo 4 as a class in 2027 (Amazon).
What happens to Mephisto?
Mephisto is not killed: he is banished. "With the help of his daughter Lilith and the weapon she had made from her own body, a single human got the better of him. Mephisto was stabbed through the heart with this weapon and banished screaming into the Void between realms. Not dead, not alive, and unable to return to Hell. There he will remain" (Blizzard's text). That matters because, as the same text says, his soul had always been "reborn in Hell" after every earlier defeat. IGN's review opens with the line "Mephisto, you shall darken the world no longer" over its footage. The single human is the Wanderer, the player. Mephisto is not "dead" and not "back in Hell"; he is in the Void, the prison Lilith once escaped. More is on Mephisto.
Lilith's role
Lilith, the villain of the base game, is the key to beating her father. Blizzard's campaign summary says "Lilith herself is the only way to reach" the beacon of hope, and the weapon that ends Mephisto is one she made from her own body. Grimdark Magazine's review says Lilith is woken through the Wanderer's blood link and "begrudgingly agrees to help" (Grimdark Magazine, Jun 29, 2026). At BlizzCon 2026 a Diablo 5 developer named that bond as what made the expansion: "the depth of that relationship that you get to build with Lilith" and with Mephisto, so that "you're not just a wanderer anymore". Her own last words in the base game, "Without me, you would have no victory," came true; Blizzard's text adds "(And spoiler, she was right)." Blizzard's published text does not say what becomes of Lilith afterward.
Tyrael returns
Tyrael, "ex-angel, founder of the Horadrim", returns in Lord of Hatred: Grimdark Magazine writes that the Wanderer reconnects with Lorath and Tyrael. Blizzard's own recap does not list him among the dead, and no Blizzard text read here says more about his part.
After the ending
Blizzard marked the end plainly: Season of Death Awakening (Jun 30, 2026) "descends in the wake of Mephisto's defeat", and "The Age of Hatred has ended" (Blizzard, Jun 23, 2026). The new threat is a death cult; that season's gameplay trailer opens on its sermon: "to live is to suffer", "Only in death will we find salvation." The seasons since follow "the dead returning to life and the dire purposes of the Cult of the Dead", toward a confrontation with the new Aspect of Death (Blizzard, Sep 13, 2026). Blizzard says no more Diablo 4 expansions are currently planned (Brent Gibson, Sep 2026), so the story goes on in Seasons.
How it leads to Diablo 5
Diablo 5 picks up about a century on, not straight after the ending. At its BlizzCon panel, the team says the win over Mephisto held: "what you did in defeating Mephisto, it worked. There was a golden age of sanctuary." Diablo 5 is set "100 years after everything that's happening with Diablo 4", Brent Gibson told PC Gamer (PC Gamer, Sep 12, 2026), and it opens seven years after Diablo conquers Westmarch. More is on Diablo 5.
